Stream stall watchdog: cut half-open model connections, keep partial output

The stream is now consumed via a pump thread with a stall watchdog (MATHAGENT_STREAM_STALL, default 180s): if no chunk arrives in time, the call is cut, partial output is kept with an explicit note, and the agent loop continues.
