[NOTICKET] chore: rename LLM env quad __4o -> __54m (GPT-5.4-mini)
Rifqi Hafizuddin +1: Claude Opus 4.8The generation LLM is Azure GPT-5.4-mini (deployment gpt-5.4-mini), but the settings quad was still named __4o. HF was left pointing at a real GPT-4o deployment while local ran 5.4-mini, so the same question over the same catalog produced a valid plan locally and hallucinated catalog ids (t_backlog, c_bf2b16fd1d3f) on HF — a silent wrong-model drift with no error surface. Rename all four settings fields + aliases to azureai__*__54m and update the 9 LLM call sites (guard, chatbot, orchestration, agents/planner, query/planner, slow_path/assembler, report/generator, handlers/help, handlers/problem_statement) plus the 2 eval runners that stamp the deployment name into result JSON. The __embedding quad is a separate model and is untouched. Hard rename, no __4o fallback: an environment missing the new names resolves to empty strings and fails on the first LLM call, so this class of drift fails loud instead of silently serving a different model.
