Add prompt refinement UI and fix text document verification
- Evidence-grounded boost: if evidence is relevant (>= 0.4), claims with ANY similarity (>= 0.2) or NEUTRAL/ENTAILED NLI are marked supported - Added prompt refinement suggestion panel when response is unverified: shows unverified claims and offers refined query buttons - Users can click suggested refined prompts to retry with better phrasing - Fixes false hallucination detection on PDF/text document queries Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>94d0abe
