fix: pipeline logic + heatmap per-condition sentence fix
- No_Finding suppressed when any pathological finding SUPPORTED/MISSING in X-ray (medically mutually exclusive with active findings) - mentioned_in now excludes CheXbert uncertain (-1.0) labels — reduces cross-contamination between unrelated conditions - HALLUCINATED→INACCURATE override when CheXbert explicitly positive (1.0): if text clearly mentions the device/finding, model cannot call it hallucinated - Heatmap fix: use best_si per condition (sentence that drove verdict) instead of always all_attn[0][ci] (always first sentence)fc65b2f

fix: proper error handling + honest /health + /debug endpoint
