Fix _process_array: add valid-region hard-clip after refine_mask

app.py duplicates the pipeline from detect_cave.process_image but was
missing the actual_lc/actual_rc clip added in v5. Without it, the
mask could include IR penumbra pixels that process_image correctly
strips, causing different results between CLI and Gradio Space.

cache_examples=True caused IndexError when the cached CSV had fewer
entries than the current examples list. Setting to False so examples
run on-demand instead of from a stale pre-computed cache.

- score_candidate: border contact penalty (×0.60 for 1 edge, ×0.35 for 2+)
prevents dark vegetation at image borders from outscoring contained cave voids
- process_image: hard-clip final mask to actual illumination columns after
refine_mask, removing IR vignette/penumbra reintroduced by GrabCut or smoothing
