library_search: drop results below a reranker relevance floor Running
Verified signatureOff-topic queries previously returned the least-bad chunks in the 30-source relational index (e.g. a Harry Potter chapter from Austin, Friendship, for a common-grace query, rerank score 0.0085). Results with cross-encoder score below 0.10 are now dropped, so such queries return status insufficient_evidence. Floor is configurable via PRIVATE_RAG_MIN_RERANK_SCORE and skipped when reranking is degraded or disabled. Calibration on 2026-10-04: in-scope 0.41-0.999, out-of-scope 0.0017-0.0085.
