Refine evaluation UI based on annotator feedback.
Adithya Nayak +1: CursorSimplify labels to Yes/No, show per-axis instructions in the eval UI, add a calibration example, lock annotator name after entry, use numeric count inputs, merge constraint checks, and refresh the instruction page from the guideline document.
