Extract doc_name from H1 title in markdown, add 2019 stroke guideline
- Populate doc_name from the H1 heading in the markdown file instead of the truncated filename, so meaningful titles are stored in ChromaDB - Extract doc_year from title as fallback when filename lacks a year prefix - Update AVAILABLE_DOCS in server_ui.py with new doc_name values - Add 2019 AHA/ASA Acute Ischemic Stroke guideline document - Truncate output filenames to 200 chars to avoid OSError on long titlescab4c60
