User-study questionnaires (anonymized)

questionnaire_blank.pdf is the form as fielded (12 items: five statements after each drive, an overall preference, and an optional free-text comment). driver01.pdf … driver14.pdf are the fourteen completed forms; driverNN corresponds to participant PNN in Appendix H of the paper. Two forms were completed by hand (circled answers) and are included as filled scans; the others contain form-field values. File names, PDF metadata, and contents carry no participant names or other identifying details; on every form "first drive" is the TriDrive condition and "second drive" is openpilot's driver-monitoring system, as stated in Appendix H.
